Job Description:
Under the supervision of the Center Head, the SWO II shall manage and support the welfare of residents with the primary responsibility to oversee and coordinate case management activities ensuring that each resident is provided with holistic care and support responsive to the individual’s needs. As the case manager, the SWO II shall: • Work closely with the residents, their families, and members of the multi-disciplinary team to identify/assess needs, develop care plans, and monitor progress; • Advocate for the residents’ rights; • Conduct individual sessions; • Provide crisis intervention as necessary; • Conduct admission, monitoring, pre-discharge and discharge case conferences; • Collaborate/coordinate with partner stakeholders to augment the center programs and services; • Maintain/update records/documents in the residents’ case folders.
Functions and Responsibilities:
• As the case manager, interview, assess, develop individualized intervention plans, implement intervention plans in collaboration with other disciplines to meet diverse needs of the residents.
• Advocate for the rights and welfare of the residents by participating in activities aimed at promoting their rights and welfare (ex. acting as a resource person).
• Conduct individual sessions with/provide crisis intervention to residents to ensure provision of emotional support.
• Conduct admission, monitoring, pre-discharge and discharge case conferences with the residents’ family members/significant others, center-based rehabilitation team, partner stakeholders and/or multi-disciplinary team members to review/monitor/evaluate intervention plan implementation.
• Work closely with the center-based and external multi-disciplinary team members to ensure holistic provision of services.
• Maintain/update/safekeep social work case management documents/records of residents.
• Ensure confidentiality of cases.
