Final Shortlisting: Top five highest rating but overall rating should not be less than 80%

Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 12, Revised 2017) which can be downloaded at www.csc.gov.ph;
Updated and properly accomplished Work Experience Sheet (CS Form 212 Attachment), which can be downloaded at www.csc.gov.ph;
Performance Rating in the last rating period position (if applicable);
Photocopy of Certificate of Eligibility/Rating/License (if applicable); and
Photocopy of Transcript of Records and Diploma;
Photocopy of Certificate of Employment and Clearance from previous employer/s;
